The H.O.P.E. for Health Campaign
"Hospitals Offering Prevention for Everyone"
The Problem
Over the next 10 years, deaths due to chronic disease are projected to increase by 17% . In California, 14 million adults (38%) live with at least one chronic condition (i.e. diabetes, heart disease, obesity, etc.) and more than half have multiple chronic conditions . In Los Angeles County, 80% of preventable disability and death is associated with chronic disease.
Background
The Los Angeles Chronic Disease Management Consortium (CDMC) organized in 1997 with the mission of reducing the incidence of chronic disease and improving the health of those with chronic disease. Facilitated by the National Health Foundation (NHF), an independent non-profit organization, the consortium includes five hospitals located in Los Angeles County. Together these partners successfully developed, piloted and evaluated multiple, coordinated chronic disease management programs. To-date, CDMC programs have focused on pediatric asthma, pediatric obesity, type 2 diabetes and cardiovascular disease. These programs have each demonstrated improved health outcomes for participants.
The H.O.P.E for Health Campaign
In an effort to build a widespread movement to replicate the positive results achieved by the CDMC across communities, the Hospital Association of Southern California (HASC), NHF and the CDMC are launching the H.O.P.E ”Hospitals Offering Prevention for Everyone” for Health Campaign. This Campaign seeks to unite hospitals around a shared vision to improve the health of patients and communities by emphasizing preventive care. Through involvement in this campaign, hospitals will play a critical role in helping to combat the rise in chronic disease and related medical costs by providing patients and communities access to needed chronic disease preventive education, care and services.
Hospital Participation in the Campaign
The H.O.P.E for Health Campaign is designed to build upon hospitals’ available assets and serve the unique needs of their patients and communities. Hospitals will take advantage of two options for involvement in the campaign: 1) Implement chronic disease prevention and management programs utilizing campaign curricula, or 2) Utilize the campaign evaluation process for existing chronic disease focused programs. By taking advantage of either of these options, hospitals will benefit by demonstrating a visible, measurable and meaningful impact on chronic disease in their communities. Results from all participating HASC hospitals will be presented in an annual report to highlight the hospital industries’ commitment to battling chronic disease region-wide. This information will also be used in HASC’s ongoing public advocacy and hospital image enhancement campaign to show hospitals' service to their communities in addressing critical local healthcare issues. The first program to be implemented will be the Healthy Eating Lifestyle Program (H.E.L.P.), which combats pediatric obesity.
