About the Campaign

The H.O.P.E for Health Campaign was developed by the National Health Foundation and the Hospital Association of Southern California to unite hospitals in an effort to combat the rise of chronic disease in communities. With a focus on prevention, participating hospitals are offering services to improve the health of their patients and communities.
