| Since 2000, the Good Hope Medical Foundation has been funding NHF to facilitate the Chronic Disease Management Consortium to collaborate on project planning and development with California Hospital Medical Center (CHMC), Childrens Hospital Los Angeles (CHLA), Good Samaritan Hospital, and Huntington Memorial Hospital (HMH). This facilitation has resulted in CHMC & CHLA replicating HMH’s community asthma program for their communities. The consortium’s work also resulted in three other projects currently being conducted at NHF: Healthy Eating Lifestyle Program (HELP), Type 2 Diabetes Screening and Education Program and the Asthma Website Project. |
