Cardiovascular Disease (Heart HELP)

 
Brief Description
The Heart HELP, Cardiovascular Disease (CVD) Program was developed by the Los Angeles Chronic Disease Management Consortium (California Hospital, Good Samaritan Hospital, Huntington Hospital and the National Health Foundation) to address the needs of low-income, ethnically diverse populations with the goal of reducing risk, delaying onset, and/or reducing the progression of CVD. At its core are two distinct activities: outreach and lifestyle workshops. Outreach will include community education, risk assessments, and medical referrals. The lifestyle workshops will focus on modifiable risk factor reduction, especially in the areas of nutrition, physical activity, and smoking cessation. For this program participating hospitals will partner with the Southside Coalition of Community Clinics to provide clinic patients’ access to needed CVD focused services.
