CHRONIC DISEASE PREVENTION & MANAGEMENT
For over thirteen years, NHF has worked on combating chronic disease in Los Angeles County and is a founding member of the Los Angeles Chronic Disease Management Consortium (CDMC). The CDMC is a collaboration of Los Angeles area hospitals and community based organizations dedicated to providing their communities with free and culturally appropriate chronic disease prevention and management services. Current and past CDMC members include California Hospital Medical Center, Children Hospital Los Angeles, Good Samaritan Hospital, Harbor-UCLA Medical Center, Huntington Hospital and National Health Foundation.
Based on the success of the CDMC’s programs, NHF developed the H.O.P.E for Health Campaign to help other hospitals in Southern California replicate the programs developed by the CDMC. Seven Southern California hospitals have signed on to the campaign and are currently implementing the HELP program in their communities.
