CHRONIC DISEASE PREVENTION & MANAGEMENT:
Over the past three years, NHF has worked on combating chronic disease in the Los Angeles area and is a founding member of the Chronic Disease Management Consortium (CDMC). The CDMC is a collaboration of Los Angeles area non-profit hospitals and organizations dedicated to providing their communities with free and culturally appropriate chronic disease prevention and management services. Members include California Hospital Medical Center, Childrens Hospital Los Angeles, Good Samaritan Hospital, Harbor-UCLA Medical Center, Huntington Hospital and National Health Foundation. Current programs consist of the Type II Diabetes Screening and Education Program, the planning stages of a Heart Healthy Eating Lifestyle Program (for cardiovascular disease), and the replication of HELP, a family-based pediatric obesity intervention program.
